Pain after ectopic

Hi Everyone,

Hoping someone can give me some insight here. I had an ovarian ectopic (small one) that was treated with expectant management. My hcg levels never went above 116 (to my knowledge). It took roughly 5 weeks to come down and I got my first negative test last week wednesday. My ectopic was on the right.

I’m experiencing soreness around my pelvic area and lower back, not sure if that’s normal? I never had much pain, more like pinches on my right side during my ectopic but ever since my level reached 15, I started having left sided pain and soreness around my lower back. Occasionally I get shoulder tip pain and rectal pain however I did have some free fluid on my scan.

What I wanted to know was if any of this was normal and if I needed to raise this. I have had 3 scans in total, my last two were 2 weeks apart with the first one at a hcg level of 40 and the other when it was a negative pregnancy test. Both showed a small ectopic 1-2cm away from the ovary and free fluid in my POD. could this be causing the shoulder pain? It doesn’t get worse over time, it’s just noticeable as I am aware of it.

I am so sorry to hear of your ectopic pregnancy loss,
Pain can be common post ectopic when however if you are experiencing worsening pain and you had free fluid in your abdomen to start with, I would speak to your medical team just to be on the safe side.
If the pain becomes severe or you become unwell and dizzy, please seek urgent medical advice.

Are there any updates? I am in a similar situation where I had free fluid (mild) but feeling some pelvic pinching and rectal spasms here and there, along with left shoulder pain (mild). I was diagnosed with EP 12/19/23 with my HCG at 515. Had my first dose of methotrexate. Went back for a scan and bloods 12/25/23 HCG at 590. My scan showed less free fluid but a “lesion” near my left ovary. Today is 7 days post injection and just feeling super defeated and scared. Any sensation I overthink and it is causing me to have debilitating mental health. My next scan isn’t for another 2 days. Also, does the free fluid resolve on its own?

Apologies date is not 12/19 but 12/22 for first methotrexate so not quite 7 days today.
